This form may be downloaded and completed at home, one form per child. Bring with you to registration which will be held after both masses Sunday 8/22 and all masses 8/28,29. Additional forms will be available at registration which will be in the parish hall. There is a fee of $25 per child with an additional fee of $25 for First Reconciliation/Eucharist preparation.
